Symphony Healthcare Services are looking for an enthusiastic and compassionateHealth and Wellbeing Coach to join West Somerset Primary Care Network, s...
The Health and Wellbeing Coach is a core
member of the West Somerset Primary Care Network (PCN) multidisciplinary team,
providing practical, personalised health coaching to people who would benefit
from support to improve their health, wellbeing, confidence and independence.
Working within professional boundaries and the NHS Health and Wellbeing Coach
Workforce Development Framework, the postholder will help people identify what
matters to them, develop their knowledge, skills and confidence, and make sustainable
changes that support better health and self-management.
You will work with people experiencing a wide range of challenges, including long-term health conditions, social isolation, low confidence and difficulties managing their physical or emotional wellbeing. Using a person-centred, strengths-based approach, you will empower individuals to make meaningful changes that improve their independence and quality of life.
As a member of the West Somerset Primary Care Network
multidisciplinary team, your role will involve facilitating group coaching opportunities where appropriate, connecting patients with community resources and working closely with GPs, Social Prescribing Link Workers, Care Coordinators, Clinical Pharmacists, our Complex Care Team and wider community partners.
The successful candidate will be compassionate,
reflective and self-aware, with a genuine passion for supporting people to
improve their health and wellbeing. They will enjoy working with people from diverse
backgrounds and recognise the strengths, skills and potential that exist within
every individual and community.
You will be required to provide structured, person-centred coaching through face-to-face, telephone and digital appointments, selecting the format according to patient need, accessibility and service arrangements.
You will receive and respond to referrals through agreed PCN pathways, clarify suitability for health coaching and prioritise according to patient need, risk and service criteria, as well as maintaining an active, appropriately sized caseload with clear goals, planned follow-up intervals and timely discharge or onward referral when coaching objectives have been met or the intervention is no longer appropriate.
