Harrow Home First- Specialist Physiotherapist

• Provide specialist physiotherapy assessment and develop evidence-based treatment plans for patients with complex, frailty-related conditions and multiple co-morbidities.
• Independently manage a varied clinical caseload, using advanced clinical reasoning and person-centred goal setting to maximise mobility, independence and support safe, timely discharge home.
• Work collaboratively within the multidisciplinary team and with CLCH specialist services, health and social care partners to deliver coordinated, integrated care.
• Identify and manage clinical risks appropriately, including lone-working considerations, informed consent and timely escalation of clinical concerns.
• Assess, prescribe and order appropriate specialist equipment to support safe mobility, moving and handling, and independent living at home.
• Contribute to the day-to-day running of the service, including triaging referrals, participating in/chairing multidisciplinary meetings and responding to changing service demands.
• Provide clinical leadership, supervision and education to junior colleagues, support workers and students, supporting their ongoing development.

Essential

• Diploma/degree in Physiotherapy
• Current HCPC registration
• Evidence of up to date CPD portfolio
• Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
• Member of Professional Body

Essential

• Experience post registration as a Band 5 physiotherapist
• Experience working at a junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory

• Skills in the assessment and treatment of adults with complex multi-pathologies
• Knowledge and understanding of complex medical conditions including multipathologies and falls
• Knowledge of role of Allied Health Professionals and interdisciplinary team working
• Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
• Knowledge of outcome measures and evidence-based practice
• Ability to problem solve, prioritise and demonstrate clinical reasoning
• Good communication and report writing skills, in both verbal and written format
• Basic IT skills - Microsoft Word, Power point, Excel
• Teaching of junior staff / students, peers in Inservice training setting
• Knowledge and understanding of the role of Social Services and voluntary agencies
• Involvement in Appraisal Scheme
