The post holder will develop, deliver, and evaluate health promotion initiatives that empower patients, families, and communities to manage their condition effectively, reduce health inequalities and improve engagement and access to healthcare services.
The role involves providing culturally appropriate education, supporting self-management, promoting healthy lifestyles, facilitating engagement with healthcare services, and working collaboratively with healthcare professionals, community organisations, schools and universities, and voluntary agencies.
Key deliverables will include
- Providing health education and self-management advice aimed at preventing exacerbation of the condition and minimising admissions for acute episodes to secondary care.
- Addressing social and lifestyle factors that impact condition management and overall quality of life, signposting to local resources when required.
- Building trust and rapport with patients whilst maintaining professional boundaries.
- Providing an In-reach service to secondary care.
- Developing close links with multidisciplinary colleagues across primary, secondary, and social care to enhance continuity of care for patients.
- Organisation of patient led education sessions and involvement with peer support programmes.

This vacancy is advertised on a fixed term contract basis for a period of 22 months due to the post being recruited as part of a limited term project within the Trust. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the requirement for the post to be maintained as a part of the project's implementation and the timescales associated with this.
Please note all existing NHS staff will only be considered on a secondment basis. This must first be agreed with your current line manager.
Education and Qualifications
Essential
- Degree or equivalent qualification in Health Promotion, Public Health, Health Education, Nursing, Allied Health Profession, or a related discipline.
- Evidence of continuing professional development
Experience
Essential
- Experience of delivering health promotion or patient education programmes.
- Experience working with individuals from diverse cultural and ethnic backgrounds.
- Experience working within healthcare, community health, or voluntary sector settings.
- Experience facilitating group education sessions and community engagement activities.
Desirable: - Experience working with people living with sickle cell disorder, thalassaemia, or other long-term conditions.
Skills and Knowledge
Essential
- Knowledge of safeguarding, confidentiality, and patient-centred care
- Excellent verbal and written communication skills
- Ability to engage effectively with diverse communities
- Ability to motivate and empower patients to improve self-management
- A valid full driving licence for the UK and access to a vehicle that can be used for work purposes.
Desirable: - Understanding of haemoglobinopathies, particularly sickle cell disorder and thalassaemia
Personal Qualities
Essential
- Good organisational and time management skills
- Competence in Microsoft Office and electronic patient record systems
- Ability to work independently and as part of a multidisciplinary team
