The role will support multidisciplinary team (MDT) meetings through preparation, coordination, minute-taking & follow-up of agreed actions, ensuring effective integrated care for patients with complex needs. A key responsibility will be the operational delivery of the PCN Directed Enhanced Service (DES), including activity monitoring, performance reporting & supporting quality improvement programmes. The post holder will lead the administration of the Enhanced Access service, developing and managing clinical rotas, coordinating staffing arrangements and monitoring service utilisation and performance.The role will provide comprehensive administrative support to the Clinical Director and PCN Management Board, including organising meetings, preparing agendas & reports, producing minutes & maintaining action logs. They will monitor monthly performance & contractual data, producing reports to support management targets, service development & decision-making.

The post holder will also support the ongoing development of the emerging GP Federation, assisting with project coordination, stakeholder engagement & the implementation of new services. Responsibilities include coordinating vaccination programmes, liaising with suppliers & partner organisations, & maintaining effective relationships across primary, community and voluntary sector services

The Care Co-ordinator will provide comprehensive administrative, operational and care coordination support to the Primary Care Network (PCN), working closely with member practices, the Clinical Director, Management Board and wider multidisciplinary teams to support the delivery of high-quality, patient-centred care.Care Coordination and Patient Support- Act as a key point of contact for healthcare professionals, ensuring patients receive appropriate support and timely access to services.

  • Support patients to access health improvement services, coaching, social prescribing interventions, peer support groups and community-based resources.
  • Assist in identifying patients who may benefit from enhanced support through population health management and risk stratification approaches.
  • Coordinate referrals and communication between primary care, social prescribing and voluntary sector organisations.

Multidisciplinary Team Working- Support the organisation, administration and delivery of multidisciplinary team (MDT) meetings across the Network.

  • Prepare agendas, collate relevant patient information, coordinate attendance and ensure accurate recording of discussions and actions.
  • Monitor and follow up agreed actions to ensure continuity of care and timely implementation of MDT recommendations.
  • Facilitate collaborative working between healthcare professionals to improve outcomes for patients with complex needs.

Primary Care Network Service Delivery- Provide administrative and operational support to the delivery of the Primary Care Network Directed Enhanced Service (PCN DES), ensuring contractual requirements, service specifications and reporting obligations are met.

  • Support the implementation and monitoring of Network programmes including Enhanced Access, anticipatory care, personalised care, population health management and integrated neighbourhood working.
  • Assist in the development, implementation and continuous improvement of PCN services, policies and operational processes.
  • Support the collection, validation and submission of activity data and evidence required to demonstrate achievement of PCN DES objectives and contractual performance standards.

Enhanced Access Service Coordination- Lead the administration and coordination of the PCN Enhanced Access Service across participating practices.

  • Develop, maintain and manage Enhanced Access rotas for GPs, nurses, pharmacists, healthcare assistants and other clinical and non-clinical staff, ensuring sufficient capacity to meet service demand.
  • Liaise with practices and clinicians to coordinate cover arrangements, manage rota changes and ensure service continuity.
  • Monitor utilisation, appointment availability and service performance, escalating issues where necessary to support safe and effective delivery.
  • Produce reports and data relating to Enhanced Access activity and performance requirements.

Performance Monitoring and Reporting- Monitor, analyse and report on monthly performance data across key PCN services and contractual targets.

  • Prepare routine and ad-hoc management reports for the Clinical Director, PCN leadership team and Management Board.
  • Support quality improvement initiatives through data collection, audit activity and performance monitoring.
  • Assist in identifying trends, service pressures and opportunities for improvement to support strategic decision-making.
  • Ensure accurate maintenance of records, databases and performance monitoring systems.

Governance and Management Support- Provide high-quality administrative support to the Clinical Director and PCN Management Board.

  • Organise Board meetings, leadership meetings and stakeholder forums, including scheduling, agenda preparation, circulation of papers and record keeping.
  • Produce accurate and timely minutes, action logs and follow-up documentation.
  • Support the preparation of business cases, service proposals, funding applications and project documentation.
  • Maintain confidentiality and ensure compliance with information governance, data protection and organisational policies.

Federation Development and Strategic Support- Support the ongoing development of the emerging GP Federation by providing administrative, project and operational support.

  • Contribute to the coordination of collaborative initiatives across member practices, helping to strengthen shared working arrangements and service delivery models.
  • Assist with stakeholder engagement, communications and organisational development activities associated with the Federation's growth.
  • Support the implementation of new services, contracts and programmes delivered through the Federation and wider PCN.

Vaccination and Programme Coordination- Coordinate communication with pharmaceutical suppliers, NHS organisations and participating practices regarding vaccination programmes and other Network initiatives.

  • Support the ordering, allocation and distribution of vaccines and related resources across practices within the Network.
  • Assist in planning and coordinating large-scale health campaigns and service delivery programmes as required.

General Responsibilities- Build and maintain effective working relationships with practices, partner organisations and stakeholders across the health and care system.

  • Contribute to the achievement of PCN objectives and the reduction of health inequalities across the local population.
  • Undertake any other duties commensurate with the grade and responsibilities of the post, as required by the Clinical Director, Management Board or PCN leadership team.

This role provides vital operational, administrative and coordination support across approximately 9 member practices serving a population of over 100,000 patients, ensuring the effective delivery of PCN services and the continued development of integrated, high-quality primary care.

Greater Preston Primary Care Network (PCN), part of the Heart of Lancashire Federation, brings together 9 GP practices to deliver integrated, high-quality healthcare services for its registered population.
