The successful GP will provide high-quality, patient-centred

clinical care to our registered population of approximately 5,200 patients. The

role will include face-to-face and telephone consultations, assessment and

management of acute and chronic conditions, and appropriate referral to

secondary care and other services.

Key duties

Conduct safe and effective clinical consultations, assessing

patients' medical needs and managing clinical risk appropriately.

Diagnose, treat and manage a broad range of acute and

chronic medical conditions.

Provide appropriate clinical advice, treatment, health

promotion and preventative care.

Manage patients with long-term conditions and support

ongoing monitoring and review.

Prescribe safely and appropriately in accordance with

current guidelines and practice policies.

Make appropriate referrals and liaise effectively with

secondary care, community services and other healthcare professionals.

Maintain accurate, comprehensive and contemporaneous

clinical records.

Work collaboratively with GPs, nurses, pharmacists,

healthcare professionals, administrative staff and the wider multidisciplinary

team.

Participate in clinical meetings, significant event reviews,

audit and quality improvement activities.

Support the practice in meeting its safeguarding

responsibilities and maintaining high standards of patient safety and

professional practice.

Maintain continuing professional development and keep

clinical knowledge and skills up to date.

The GP will work as part of the practice clinical team and

will be accountable to the Practice Partners/Clinical Lead.

Job purpose

To provide a high standard of clinical care to the practice

population, working collaboratively with colleagues to ensure patients receive

safe, effective, evidence-based and patient-centred care.

The post holder will

Provide comprehensive general medical services to registered

patients.

Undertake face-to-face, telephone and other appropriate

consultations.

Assess, diagnose and manage patients presenting with acute

and chronic medical conditions.

Provide appropriate clinical management, treatment and

follow-up.

Prescribe safely and effectively within professional and

practice guidelines.

Make appropriate referrals and seek advice where required.

Provide appropriate preventative care, health promotion and

screening.

Participate in the management of patients with long-term

conditions.

Maintain accurate and timely clinical documentation.

Respond appropriately to urgent and clinically significant

presentations.

Work effectively with the wider multidisciplinary team.

Participate in practice meetings and clinical governance

activities.

Contribute to audit, significant event analysis and quality

improvement.

Maintain professional knowledge and competence through

continuing professional development.

Comply with practice policies, information governance

requirements, safeguarding procedures and relevant professional standards.

Maintain patient confidentiality at all times.

Undertake any other reasonable duties appropriate to the

role.

Working relationships

The GP will work closely with practice partners, GPs,

practice nurses, healthcare assistants, pharmacists, social prescribers,

administrative staff and other members of the primary care and community teams.

Clinical governance

The post holder will contribute to maintaining high

standards of clinical governance, patient safety and quality improvement within

the practice and will participate in relevant practice meetings, audits and

reviews.

Qualifications

Essential

  • MBBS or equivalent medical qualification.
  • Full registration with the General Medical Council.
  • Inclusion on the GMC GP Register.
  • Inclusion on the NHS England Medical Performers List.
  • MRCGP or equivalent recognised GP qualification.
  • Appropriate postgraduate training in General Practice.

Desirable

  • Experience or interest in a specific clinical area relevant to the practice.
  • Experience of clinical audit or quality improvement.
  • Experience of teaching, mentoring or supporting other healthcare professionals.
  • Experience of working within multidisciplinary primary care teams.
  • Additional postgraduate qualifications or relevant clinical skills.
  • Previous experience of contributing to service development.
