- Provide General Medical Services (GMS) in accordance with the NHS GMS contract
- Undertake surgery based consultations, telephone consultation and home visits
- Management of patients with acute and chronic conditions
- Prescribe safely and appropriately in line with practice and national guidelines
- Process and action results, clinical correspondence and referrals
- Maintain accurate, contemporaneous clinical records in line with professional guidelines
- Participate in weekly practice meetings, clinical governance, audits and quality improvement activities
- Supervision of medical students and resident doctors where required
- Work collaboratively as part of our multidisciplinary team
The GP will provide high-quality, patient-centred General Medical Services to the registered patients of Ystwyth Medical Group. The successful candidate will work as part of our multidisciplinary team to deliver safe and effective healthcare, whilst supporting the practice's commitment to continuity of care, medical education and service development.
Undertake routine and urgent GP consultations, appointments including home visits and an on call requirement
Assess, diagnose and manage acute and chronic medical conditions
Prescribe safely and appropriately in line with local and national guidance
Make referrals to secondary care and community services where appropriate
Review pathology, radiology and other investigation results
Undertake home visits where clinically required
Provide advice and support to patients and carers
Participate in safeguarding processes for children and vulnerable adults
Maintain accurate and contemporaneous clinical records using EMIS
Complete referrals, reports, correspondence and associated clinical administration
Review laboratory and diagnostic results
Action workflow tasks within agreed timescales
Complete medical reports and documentation as required
Participate in clinical governance activities
Contribute to quality improvement and audits
Comply with GMC standards and professional requirements
Maintain up to date mandatory training and appraisal requirements
Participate in Significant Event Analysis and learning activities
Participate in Practice, LHB and Cluster PT4L sessions
As a teaching practice, opportunities are available to:
Support undergraduate medical students
Contribute to and support Foundation Doctor training
Contribute to and support postgraduate GP training
Participate in educational activities and mentoring where appropriate
Work collaboratively with GP Partners, Salaried GPs, Practice Nurses, HCSWs, Advanced Clinical Pharmacists and Nurses and the wider multidisciplinary team
Contribute positively to team development and service improvement

We are a high achieving practice operating from a purpose-built premises and providing high quality care to our 9300 patients, using the EMIS clinical system.
Our thriving, multi-disciplinary practice comprises of a diverse clinical team including 3 GP Partners, 4 Salaried GPs, Advance Clinical Pharmacist prescriber, Advanced Nurse Practitioner and highly professional nursing and administrative support teams.

Qualifications
Essential
- Fully qualified General Practitioner
- Registered with the General Medical Council (GMC) with a licence to practise
- Included on the NHS Wales Performers List or eligible for inclusion
- Evidence of ongoing CPD
- Excellent communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Commitment to high-quality patient care
Desirable
- Interest in medical education and training
- Experience of EMIS
- Interest in developing specialist clinical areas
- Previous experience working within UK General Practice
