And Responsibilities
Scan
patient related documents onto their medical record.
Ensure
incoming letters are scanned onto the patient record
Identify
and appropriately action relevant clinical information contained in patient
correspondence.
Code
or free text the clinical information on to the patient record.
Summarisation
of patients notes when requested.
Ensure
security of data at all times.
Process
patient records for Medical Reports etc
Dealing
with patients for access to medical records.
Create
patient online registrations, generating access levels and dealing with
day-to-day enquiries.
To
be responsible for patient records coming in and out of the practice, in
accordance with practice procedures.
Processing
and removing deceased patients and out-of-area patients.
Processing
new patient registrations.
Reviewing
medical records and producing an accurate summary of the patients medical
history, read coding information in line with procedures.
Sort
all incoming patient related mail.
Scan
patient related documents onto their medical record using read codes as agreed
by the clinical team.
Code
the clinical information on to the patient record.
To
provide an efficient copy/audio typing and word processing service for all
Clinicians as required.
To
make appointments and bookings for patients as required.
Provide
secretarial support to Palliative Care and Operations meetings.
Support
the Care Navigation Team a rota basis.
Duties and Responsibilities
Sort (in date
order) all incoming patient related mail.
Scan patient
related documents onto their medical record using read codes as agreed by the
clinical team.
Add any additional
information about the source of the document.
Forward the
document to the doctor or nurse who initiated the referral or who is most
appropriate to receive the information.
Ensure incoming
letters are scanned onto the patient record within a 48/72 hour timescale.
Identify and
appropriately action relevant clinical information contained in patient
correspondence
Code or free text
the clinical information on to the patient record
Deal with queries
relating to coding
Filing
and retrieving paperwork
Summarisation
of patients notes when requested.
Ensure
security of data at all times
Patient Experience
Support the Care Navigation Team and respond to
patient calls between 08.00 10.00 on a rota basis.

Postholders will be expected to undertake any work that comes within
the remit of the posts purpose. The
job description will be kept under review and may be changed according to Practice
requirements. Discussions on any major
changes will be held with the postholder.

Qualifications
Essential
- GCSE grade A to C in Maths and English
Desirable
- Previous experience in general practice especially coding
