The post-holder will make themself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home and in Care homes, checking and signing/authorising repeat
prescriptions and dealing with queries, paperwork, and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards.
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
Visit patients/care homes during surgery time in an emergency if clinically indicated.
To undertake a fair proportion of on call sessions per week as allocated on the PCN rota.
The post-holder will undertake fair share of home visiting sessions per week as allocated.
Patient Management
- Create opportunities to address patients care needs in the
most expedient way, taking into account that continuity of care and access to
services are often in direct conflict.
- Encourage patients to be aware of the service pressures
within primary care and to manage their expectations of what we can
realistically deliver in a manner that is fair and proportionate for all care
priorities.
- Make improved use of the telephone and other technologies.
Encourage patients to rely on face-to-face consultation only where there is a
clinical need to do so.
- Communicate and reassure patients of the need to offer
alternative care options where appropriate and safe to do so.
- To immediately highlight any concerns, you may have in
respect of safety or quality of patient care to the Clinical Director and
Practice Partners.
- Monitor and evaluate adherence to local chaperoning
policies
Confidentiality
- In the course of seeking treatment, patients entrust us
with, or allow us to gather, sensitive information in relation to their health
and other matters. They do so in confidence and have the right to expect that
staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this job
description, the post-holder may have access to confidential information
relating to patients and their carers, practice staff and other healthcare
workers. They may also have access to information relating to the practice as a
business organisation. All such information from any source is to be regarded
as strictly confidential.
- Information relating to patients, carers, colleagues, other
healthcare workers or the business of the practice may only be divulged to
authorised persons in accordance with the Practice/PCN policies and procedures
relating to confidentiality and the protection of personal and sensitive data.
- The post-holder should respect patient confidentiality at
all times and not divulge patient information unless sanctioned by the
requirements of the role.
- The post-holder will comply with all relevant information
governance, confidentiality and data protection requirements, including the UK
General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, the
Access to Health Records Act 1990 where applicable, and any subsequent or
amending legislation, together with relevant NHS information governance
policies and standards.
Data Protection and Employee Information
As your employer, the Practices/PCN will need to collect, use and retain
personal information about you for purposes connected with your employment.
This may include information required for payroll and pension purposes, contact
and emergency contact details, references, qualifications, training records and
records relating to your employment and career with the PCN.
Your personal information will be processed in accordance
with the UK General Data Protection Regulation (UK GDPR), the Data Protection
Act 2018 and other applicable data protection legislation. Information will
only be collected, used, retained and shared where there is an appropriate
lawful basis for doing so and in accordance with the Practices'/PCN's relevant
data protection and privacy policies.
Managing information
- Use technology and appropriate software as an aid to
management in planning, implementation, and monitoring of care, presenting and
communicating information.
- Review and process data using accurate coding to ensure
easy and accurate information retrieval for monitoring and audit processes.
- Understand responsibility of self and others to the
practice and PCN regarding the Freedom of Information Act
2000.
- Makes accurate record keeping in EMIS and makes effective
use of the clinical system by ensuring the consistent and accurate entering of
data and use of SNOMED Codes.
- Facilitates the use of up-to-date information technology to
collate accurate and timely information as and when required by the practice.
- Works within the Caldicott and Data Protection principles.
- Assist in the development of practice protocols for the
practice and clinical protocols for the clinical team.
- Alert other team members to issues of quality and risk.
Information Security
All staff are responsible for ensuring that personal and
confidential information relating to patients, staff and other individuals,
whether held electronically or in paper form, is handled securely and
confidentially at all times. Staff must comply with the UK General Data
Protection Regulation (UK GDPR), the Data Protection Act 2018, other applicable
data protection legislation and the Practices'/PCN's information governance,
confidentiality and information security policies and procedures. Computer passwords
must not be shared either between systems or users. The Practices/PCN may undertake appropriate
monitoring of organisational systems, networks, devices and communications for
legitimate purposes, including information security, system management and
compliance with organisational policies. Any monitoring will be undertaken
lawfully, transparently, necessarily and proportionately, in accordance with
applicable data protection legislation and the Practices/PCNs relevant
policies and privacy information.
Access and usage of practices/PCNs computers must be in
accordance with the Practices/PCNs Policies. Approved secure methods
/procedures are to be used for all electronic transfer of personal data. This
is in order to protect the practices, patients and staff, and its reputation
and to ensure that it complies with the laws and other guidelines.
Records Management
As an employee you are legally responsible for all records
that you gather, create, or use as part of your work within the Practices/PCN,
and they remain the property of the Practices/PCN. This includes patients,
financial. personal and administrative records, whether paper based or on
computer. you have a legal duty of confidence to all service users and you should consult and ask for guidance from
your manager if you have doubt about the correct management of records with
which you work. All staff have a responsibility to ensure information quality
standards are achieved.
Training/Meetings
- Attending training and events
organised by the Practices/PCN or other agencies, where appropriate.
- Participate positively and
actively in clinical and Practice/PCN meetings.
- The PCN and Practices have a
strong ethos around training and development of others, therefore the
successful candidate will be willing and able to support medical students, and
other training roles.
Personal/Professional
Development
- Participation in an annual
individual performance review, including taking responsibility for maintaining
a record of own personal and/or professional development demonstrating skills
and activities to others who are undertaking similar work.

Our PCN staff currently includes Clinical Pharmacists, Social Prescribers, Health & Wellbeing Coach, Mental Health Practitioner, Paramedics, First Contact Physiotherapist, GPAs, Pharmacist Technicians, PCN Manager/Digital & Transformation Lead and a Care Home Team.
Experience
Essential
- Qualified GP
- MRCGP
- Vocational Training Certificate or equivalent JCPTGP
- General Practitioner (Certificate of Completion of Training CCT)
- Full GMC registration
- National Performers List registration (England), noting that
- confirmation on this list is adequate for DBS confirmation
- Appropriate defence indemnity (MPS/MDU)
- Evidence of current validation
- Evidence of last appraisal
- Eligibility to practice in the UK independently
- Experience of working in a primary care environment
- Experience of continued professional development
- Experience of QOF and clinical audit
- Not previously been substantively employed at a Core member practice in last 12 months
- General understanding of the primary care contract
- Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP
- Excellent communication skills (written and oral)
- Strong IT skills
- Clear, polite telephone manner
Desirable
- Experience of medicines management
- Experience of ICB initiatives
