We are seeking to appoint a number of GPs with an interest in frailty to support the delivery of various innovative Frailty Services as part of the No...

The postholder will undertake a variety of

duties, primarily providing clinical advice and support to system providers

regarding vulnerable patients in the community as part of a multidisciplinary

team.

Key responsibilities include

  • Delivering high-quality community care for frail and vulnerable patients
  • Undertaking clinical assessments, home visits, and autonomous decision-making
  • Coordinating multidisciplinary care to support patients to remain safely at home
  • Managing investigations, referrals, prescribing, and accurate clinical documentation
  • Working collaboratively with MDT colleagues across health and social care services
  • Supporting training, professional standards, and ongoing frailty-focused CPD

Skills and Competencies

Essential

  • IT Skills (MS office / Systmone / EMIS)
  • Advanced clinical assessment skills including history taking and clinical examination
  • Excellent communication and interpersonal skills
  • Ability to demonstrate problem solving with effective decision making skills, supported by a best practice approach
  • Able to work autonomously and as part of a team
  • Able to prioritise effectively
  • Able to work under pressure

Desirable

  • Evidence of Statutory and Mandatory Training

Qualifications

Essential

  • Fully Qualified Doctor with current Professional Registration (GMC)
  • MRCGP Qualification

Desirable

  • Evidence of CPD
  • Diploma in Geriatric Medicine Or equivalent

Experience

Essential

  • Experience of Triage / Telephone Assessment
  • Up to date knowledge of the latest guidelines and evidence in older peoples medicine

  • Experience in Geriatric Medicine

  • Fully Qualified Doctor with current Professional Registration (GMC)
