The postholder will undertake a variety of
duties, primarily providing clinical advice and support to system providers
regarding vulnerable patients in the community as part of a multidisciplinary
team.
Key responsibilities include
- Delivering high-quality community care for frail and vulnerable patients
- Undertaking clinical assessments, home visits, and autonomous decision-making
- Coordinating multidisciplinary care to support patients to remain safely at home
- Managing investigations, referrals, prescribing, and accurate clinical documentation
- Working collaboratively with MDT colleagues across health and social care services
- Supporting training, professional standards, and ongoing frailty-focused CPD

Skills and Competencies
Essential
- IT Skills (MS office / Systmone / EMIS)
- Advanced clinical assessment skills including history taking and clinical examination
- Excellent communication and interpersonal skills
- Ability to demonstrate problem solving with effective decision making skills, supported by a best practice approach
- Able to work autonomously and as part of a team
- Able to prioritise effectively
- Able to work under pressure
Desirable
- Evidence of Statutory and Mandatory Training
Qualifications
Essential
- Fully Qualified Doctor with current Professional Registration (GMC)
- MRCGP Qualification

- Evidence of CPD
- Diploma in Geriatric Medicine Or equivalent

- Experience of Triage / Telephone Assessment
- Up to date knowledge of the latest guidelines and evidence in older peoples medicine
