Frailty Advanced Clinical Practitioner

• To work alongside the Consultant Practitioner, Consultant Geriatricians, Head of Nursing, Lead Nurses, Head of Therapies, Matrons and external partners to further develop Frailty pathways, including the acute Frailty service in ED, inpatient frailty liaison, community integrated care teams, nursing homes and with the Frimley Health and Care ICS.
• To provide expert high-quality person-centred care which always considers people's safety, privacy and dignity using advanced clinical knowledge that is evidence based.
• As part of a MDT, to practice autonomously, whilst being accountable and self-directed in-line with their relevant code of professional practice.
• To undertake Comprehensive specialist holistic assessments for individuals presenting with frailty within the acute care setting, with the purpose of creating an individualised agreed management plan; incorporating pharmacological considerations, long-term conditions management, frailty syndrome identification, prevention and management, diagnostic reasoning and initiating onward referral to specialist teams as appropriate.
• To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision-making skills.
• To lead as a driver for change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research and education.

Essential

• Registered General Nurse/ Registered Mental Health Nurse/Practitioner/ Registered Pharmacist / Registered AHP
• Post registration qualification or equivalent experience gained by undertaking on-going personal development and training focused on older adults / frailty / advanced practice skills.
• Master's in Advanced Practice or Equivalent ACP credentialed
• Registered V300 Non-Medical Prescriber with evidence of updating knowledge appropriate to patient group

• Management /Leadership Qualification or development programme

Essential

• Management/Leadership Experience
• Evidence of involvement in the development of programmes of care, protocols and audits.
• Significant post registration experience at band 7 or equivalent working with Older Adults
• Clear and demonstrable evidence of advanced, autonomous practice working with the older adult population o
• Evidence of proactive involvement in the supervision and development of members of the multi-disciplinary team and able to demonstrate changes of practice through these
• Evidence of involvement in successful change management
• Experience in co-ordinating and leading Comprehensive Geriatric Assessments with the Older Adult population living with Frailty

Desirable

• Experience of caseload management including responsibility for complex care packages for vulnerable people
• Evidence of research involvement

Essential

• Ability to demonstrate specialist expertise and knowledge underpinned by theory acquired through CPD in relation to history taking, clinical assessment, planning, implementation and evaluation of care
• Knowledge and understanding of the Trust Strategy relevant to the role
• Ability to demonstrate specialist clinical reasoning skills to assimilate information in order to make a clinical judgement regarding diagnosis and intervention
• Demonstrates an understanding and insight into current issues and developments within the NHS
• Confident in the use of computer systems, spread sheets, databases, data collection, word processing and report writing

• Ability to demonstrate innovation and effective use of resources in practice
• Ability to actively search for the potential to improve quality and customer care
