Manage a caseload of service users, undertaking assessment, care planning, risk management and regular reviews in line with Trust policies and recovery-focused practice. Deliver evidence-based interventions, including psychosocial and CBT-informed approaches, tailored to individual needs.

Work collaboratively with service users, carers and families to promote recovery, choice, independence and personalised care. Monitor mental and physical health needs, ensuring appropriate interventions and support are provided.

Participate in multidisciplinary meetings, case discussions, caseload management and the Shared Care/Duty rota. Maintain accurate records and communicate effectively with colleagues, service users and partner agencies.

Provide supervision, guidance and support to support workers, junior staff and students as required. Contribute to safeguarding, risk management and quality improvement activities, escalating concerns appropriately.

Work autonomously to prioritise workload while engaging in regular supervision, reflective practice and continuing professional development. Maintain professional registration, discipline-specific competencies and mandatory training requirements.

The successful candidate will be a qualified Nurse, Social Worker or Occupational Therapist with excellent assessment, communication and organisational skills, and a commitment to recovery-focused, person-centred care.

Qualifications

Essential
