o Work collaboratively within the Integrated District Nursing Team and wider multidisciplinary team to deliver high quality, coordinated, and patient centred care across the community.
o Assess, plan, implement, and evaluate care in partnership with patients, families, and carers, ensuring personalised care plans reflect individual needs and preferences.
o Support the delivery of safe, effective, and responsive community nursing services, promoting independence, wellbeing, and positive patient outcomes.
o Contribute to service improvement and quality initiatives, supporting the ongoing development of community services to meet the changing needs of the local population.
o Deputise for the Team Leader as required, providing day to day clinical and operational leadership and ensuring continuity of service delivery.
o Support the supervision, development, and wellbeing of team members, promoting a positive learning culture and high standards of clinical practice.
o Work in partnership with primary care, acute services, social care, and other healthcare providers to ensure seamless care pathways and effective multidisciplinary working.
o Contribute to clinical governance, risk management, and quality assurance activities to support continuous service improvement and excellent patient care.

Education/Qualification
Essential
- RGN / RN with current registration
- Evidence of recent and continuing post-registration professional education and training
- Practice supervisor/assessor training
Desirable: - Has undertaken formal training in staff and team management; - Nurse prescribing qualification
Experience
Essential
- Evidence of recent post-registration nursing experience
- Experience of negotiating and liaising within a multi-disciplinary team
- Experience of working with people with complex health needs
- Experience of supervising junior staff and co-ordinating a team on a day to day, regular basis
- Experience of teaching students and other staff members
- Experience of implementing evidence-based care in practice
- Experience of undertaking comprehensive health needs assessment and writing care plans
Desirable: - Experience of managing staff; - Experience of managing staff Post-registration community nursing experience
Skills & Knowledge
Essential
- Principles of care for patients with long term conditions, based on local/national guidelines and standards
- NMC Code of Professional Conduct and application to practice
- Appraisal and Personal Development Planning
- Understanding of NHS and Integrated Care boards structures
- Leading audit process
- Applying evidence based care into practice
- The principles of clinical governance and application to practice
- Clinical supervision - purpose and application
- How equal opportunities can be implemented in practice
