Qualifications, Specific Experience & Training
Essential
- Registered general nurse (RGN) educated to degree level (or equivalent experience)
- Current NMC
- Evidence of continuous professional development
- Evidence of having a managerial qualification or willingness to undertake courses available through the Trust to work towards one.
Desirable: - Preparation for Mentorship and Preceptorship qualification (or equivalent experience)
Knowledge and Skills
Essential
- Significant post registration nursing experience
- Previous experience of co-ordinating hospital discharges including individual care package plans in an acute hospital setting
- Experience of chairing case conferences
Personal Attributes
Essential: - Good IT skills; - Good planning and organisational skills
Desirable: - Ability to travel across locations
