Complex Care Manager

The post holder will hold delegated responsibility for a defined complex care portfolio (inner or outer north east London, babies, children and young people (BYCP) or Adults), leading the commissioning, review and quality assurance of individual packages of care. Working within a system governance framework, the role will ensure that packages deliver high-quality, person centred outcomes, support de-escalation where appropriate, and represents value for money, while contributing to wider service and pathway development. The postholder will be responsible for ensuring the ICB is appropriately contributing to the development of Dynamic Support Registers and any successor arrangements. The team will be required to keep abreast of changing legislation and statutory guidance and to ensure their work is informed by NICE guidelines and by best practice. They will need to work closely with local government colleagues to build consistent and coherent joint commissioning of individual packages of care, with Place Delivery Teams and with the Heads of SEND co-ordination.
