Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.
- Assess the patients home to ensure that they are not injured and it is safe for you to assist
- Support the patient in regaining an upright position, with the support of a multitude of equipment
- Support the patient with any basic care requirements
- Complete manual handling as appropriate for the scenario and in line with manual handling guidelines
- Complete the IStumble evaluation form using our EMIS system for the patient
- Complete the patient referral in its entirety
- Complete any onwards referrals for the patient
- Add any follow up calls & complete when necessary
- Be part of the whole CRH Team
- Take phone calls from community services, patients and other HCP's and ASC professionals and complete the referrals on the EMIS system
- Being able to multi-task and using an array of different systems
- Using a computer will be a big part of your role
- Being able to either touch type or type confidently at a steady speed
- Understanding that you are part of a much wider team
- Being adaptive and moving quickly with new processes that are implemented
- Having a positive and a can do attitude, especially when face to face with the patients.
- Friendly and confident telephone manner

Education & Qualifications
Essential
- CPD Diary effectively demonstrating levels of learning and reflection from courses / experiences
- Experience of working in a Health/Social Care/related environment
- Experience of working in a community setting
Desirable
- Care certificate
- Manual handling
Skills, Knowledge & Abilities
Essential
- Awareness of Health and Safety and Manual Handling requirements
- Good communication skills.
- Experience of and ability to use IT programmes
- including Microsoft Office products
- Being able to multi task effectively
- Experience of being able to prioritise tasks

- Supporting patients with onwards referrals
