Community Staff Nurse

To promote the health and wellbeing within the family

unit and the community as a whole.

Perform comprehensive assessment of patient nursing

needs as the named nurse.

Plan, assess

and evaluate programmes of care according to changing health needs under the

direction of the District Nurse.

To support the team leader in the

co-ordination of complex and continuing care packages.

Maintain accurate records in accordance

with NMC.

Attend a multi-disciplinary case

conference as the named nurse if appropriate.

To communicate and collaborate with other members of

the PHCT, voluntary and statutory organisations to support individual patient

care.

To establish and maintain effective communication

with patients, relatives and carers in complex and potentially challenging

situations.

To competently perform clinical skills related to

the post.

To be professionally responsible for a delegated

caseload under the supervision of the District Nurse.

To assess health needs at individual, practice and

community level.

To undertake chronic disease management reviews

using appropriate template.

work without direct supervision

The post holder will play an essential role within the

District Nursing Team by delivering high-quality, patient-centred care under

the supervision of the District Nurse. They will undertake comprehensive

assessments of patients nursing needs, develop and implement individualised

care plans, and continuously evaluate the effectiveness of care, ensuring that

all interventions respond to patients changing health needs. Close liaison

with the District Nurse is expected to ensure seamless communication, continuity

of care, and ongoing review of complex or continuing care packages.

The role involves regular communication and collaboration

with a wide range of professionals and agencies, including members of the

Primary Health Care Team, voluntary organisations, and statutory services. This

multidisciplinary approach ensures coordinated, holistic care for patients and

supports the wider health and wellbeing of individuals, families, and the

community. The post holder will maintain accurate and contemporaneous records

in line with NMC requirements and may attend multidisciplinary case conferences

as the named nurse when appropriate.

They will demonstrate effective communication skills when

working with patients, relatives, and carers, including in complex or sensitive

situations. Competence in a range of clinical skills is required, and the post

holder will independently manage their workload, performing chronic disease

management reviews, assessing health needs at individual and community levels,

and delivering care without direct supervision. A key part of this role is to

contribute to teaching, mentoring, and assessing students and junior

colleagues, supporting their development and helping to ensure a skilled and

confident workforce.

From a managerial perspective, the post holder will seek

guidance from the District Nurse Coordinator or a District Nurse when the

designated District Nurse is unavailable, ensuring safe oversight of day-to-day
