This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex nursing needs within the community setting.
This is an excellent opportunity for dedicated and committed registered nurses who are looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long term conditions.

Qualifications
Essential: - Level 1 registered nursing qualifications
Desirable: - Post registration qualification in teaching/assessing; - Evidence of continuous professional development to degree level or equivalent experience
Experience

- Demonstrable recent Nursing experience.
- Demonstrate evidence to motivate team/peers.
- Evidence of clinical/professional development.

- Demonstrable understanding of the extent of the role.
