An exciting opportunity has arisen for the recruitment of a Community Staff Nurse based at Great Eccleston Nursing Team. The advertised role is a Temp...

The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.

To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.

The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.

To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.

Assess, plan, implement and evaluate the individual requirements of patients and carers referred to District Nursing

Provide a high standard of evidence based nursing care to patients in the Community.

Use a high level of communication skills with patients and their families, including those with English as a second language or learning disabilities.

Actively seek to promote health both individually and to patients and carers.

Administer drugs within NMC guidelines.

Education and Qualification

Essential

  • Registered Nurse level 1
  • Evidence of ongoing professional development
  • Current NMC registration
  • Supervisor/Assessor competence or willingness to undertake

Desirable

  • V150/V100/V300 Prescriber
  • Evidence of CPD - Management of Leg Ulcers CPD Module
  • Evidence of CPD - Tissue Viability CPD module

Experience and Knowledge

Essential

  • Post registration experience
  • Able to deal with difficult situations
  • Ability to Identify and assess patients' needs
  • Ability to use resources appropriately
  • Multidisciplinary working
  • Willingness to undertake further training specific to role

Desirable

  • Previous community nursing experience
