Job summary Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maximise their independence and delay or avoid ongoing care, helping them stay at home. Our team, including Community Reablement Assistants, Occupational Therapists and Sensory Officers, collaborate with Health colleagues to support those with short or long-term conditions and those transitioning from hospital.
