The post holder will screen referrals into the Crisis Access Hub using agreed protocols, clinicalrecordsand information from referrers to support safe pathway decisions. You will contribute to the 1-hour screening target by managing your own referral workload, documenting decisions clearly and escalating delays,complexityor uncertainty to the Band 7 Senior Clinical Practitioner. You willidentifyrisk, safeguardingconcernsand immediate safety issues, apply the trusted assessor principle whereappropriate, and route referrals to the right onward service. The role requires sound clinical judgement, clear communication,accuratedocumentationand the ability to work calmly in a high-demand crisis access environment.
