- Deliver high-quality, patient-centred clinical pharmacy services within a busy, forward-thinking primary care team.
- Manage acute and repeat prescription requests, ensuring prescribing is safe, effective, evidence-based and clinically appropriate.
- Conduct structured medication reviews (SMRs) and support patients with long-term and complex conditions, including chronic pain, hypertension and diabetes.
- Provide patient consultations and comprehensive reviews of clinical notes, medicines and prescribing records, applying NICE guidance, QOF requirements and local prescribing policies.
- Work closely with the wider multidisciplinary team (MDT), providing expert pharmaceutical advice and contributing to complex clinical decision-making.
- Support the safe management of shared care agreements, ensuring continuity and safety across services.
- Supervise and provide clinical support to pharmacy technicians and contribute to the training and development of trainee pharmacy technicians and pharmacists.
- Contribute to clinical audits, quality improvement and service development initiatives.
- Use strong clinical judgement and excellent time-management skills to work effectively under pressure while maintaining high standards of patient care.
- Bring up-to-date clinical knowledge and a proactive approach to improving medicines optimisation across the practice.
Experience with SystmOne is desirable.

We are looking for someone who is proactive and patient-focused, with the ability to work independently while also being an enthusiastic and collaborative member of the wider team.
As a Clinical Pharmacist, you will play a key role in ensuring the safe, effective and evidence-based use of medicines across the practice. You will work across both patient-facing and clinical activities, using your professional judgement to identify opportunities to improve treatment, support better outcomes and promote medicines optimisation.
Your responsibilities will include
- Managing acute and repeat prescription requests, ensuring prescribing is safe, appropriate, cost-effective and consistent with current national and local guidance.
- Delivering Structured Medication Reviews (SMRs) and providing personalised support to patients with long-term and complex health needs.
- Working with patients who may have multiple conditions or medicines, including those living with chronic pain, hypertension and diabetes.
- Conducting patient consultations and reviewing clinical records, medication histories and prescribing to identify risks, optimise treatment and improve outcomes.
- Applying current NICE guidance, QOF requirements and local prescribing policies to support high-quality, evidence-based clinical care.
- Providing specialist pharmaceutical advice to GPs, nurses and other members of the multidisciplinary team.
- Contributing to clinical discussions and supporting the MDT with complex medication-related decisions.
- Supporting the safe and effective processing and management of shared care agreements, helping to ensure continuity of treatment between primary and secondary care.
- Providing clinical supervision, guidance and support to pharmacy technicians within the practice.
- Supporting the learning and development of trainee pharmacy technicians and trainee pharmacists, helping to create a positive environment for professional development.
- Participating in clinical audits, quality improvement and service development projects, identifying opportunities to improve patient safety, medicines use and the overall quality of care.
- Keeping clinical knowledge and professional practice up to date and actively contributing to the development of the practice pharmacy service.

Experience of working in primary care would be advantageous but not essential, and familiarity with SystmOne is desirable.

We are a friendly, well organised and social surgery with an
increasing patient list of 15,550 patients, operating from three sites; North
Shields, Hadrian Park and Tynemouth.
We are a teaching practice with GP Registrars, F2 Doctors,
Physician Associate, pharmacy and medical students on placement.
Our clinical team comprises; 6 GP Partners, Salaried GPs, Practice Nurses, HCAs, Physician Associate, Advanced Paramedic Practioner, 3 Pharmacists and 3 Pharmacy
Technicians.
The practice hosts First Contact Physio clinics and has an
in-house Mental Health Nurse.
CQC rating as Good with high levels of QOF achievement
