This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.
- Clinically assessing, diagnosing, and prescribing (where qualified)
- Optimising medicines and improving prescribing safety
- Collaborating with a multidisciplinary team to enhance patient outcomes
- Supporting medication reviews and structured medicines optimisation
- Contributing to service development and innovative working practices
- Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities

- Knowledge of local and national healthcare priorities
Qualifications
Essential
- Masters degree in pharmacy (MPharm) or equivalent recognised pharmacy qualification.
- Registered as a Pharmacist with the General Pharmaceutical Council (GPhC), with no restrictions on practice.
- Independent Prescriber qualification (essential)
- Completion of Primary Care Pharmacy Education Pathway (PCPEP) qualification (essential)
- Evidence of continuing professional development (CPD) relevant to clinical pharmacy practice.
Desirable: - Postgraduate clinical pharmacy qualification (e.g. Clinical Diploma or equivalent).; - Membership or Fellowship of the Royal Pharmaceutical Society (RPS).
