The post holder will
- Provide specialist palliative care for patients with life limiting illnesses in community settings.
- Develop partnerships with members of Multidisciplinary Team in the Trust in order to provide a seamless service of palliative care, to enable the patient to be maintained in an environment of their choice.
- Provide clinical professional support, resources and education when necessary to members of the Multidisciplinary Team.
- Work as an autonomous practitioner within a multidisciplinary environment, ensuring adherence to professional codes of conduct.
- Work with other professionals to promote and develop the ethos of Palliative Care within the Trust and the local community.
The post holder must be able to demonstrate that they have the ability to work as an integral member of the multidisciplinary team and have excellent leadership and communication skills. They will demonstrate management of people with complex life limiting illnesses and will work in partnership to enable patients to be cared for in their preferred place of choice. Substantial palliative care experience would be an advantage.

The successful candidate will be a vital member of the Palliative Care Team across both North Tyneside and Northumberland which is a large multidisciplinary service including Consultants, Nurses, Occupational Therapist, Physiotherapists, Therapy Assistants and Nursing Assistants.
Qualifications
Essential
- Registered general nurse with substantial post registration experience.
- Diploma/Degree level of study
- Teaching Qualification (ENB 998 or C&G 730 or equivalent)
- Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period unless post is bank only).
- Educated to at least a degree level or equivalent experience/knowledge.
Desirable
- Supplementary prescribing qualification.
- Diploma in Palliative/Cancer Care.
- Post-registration palliative care qualification/experience
Experience and knowledge
Essential
- Significant post registration experience.
- Relevant experience in Palliative/Cancer Care working at Bands 5/6 or equivalent. Symptom management.
- Knowledge and experience of the audit cycle.
Desirable
- Knowledge of or experience in coaching and mentoring practices and tools
- Knowledge of or experience in Quality improvement tools, techniques and methods
Other requirements
Essential
- It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
