Clinical Lead/Manager

We require a motivated, compassionate and enthusiastic leader for our Frailty Same Day Emergency Care service. The service provides timely multidisciplinary assessment and intervention for people living with frailty, supporting admission avoidance, early supported discharge and safe transfer of care.

Proven leadership, excellent interpersonal skills and the ability to influence, support and motivate others are essential to this role.

Provide a focused leadership role to manage staff and activities within Frailty SDEC.

Act as a visible role model and maintain the highest standards of patient care.

Lead and coordinate the nursing team within the service, demonstrating compassionate leadership and effective role modelling.

Work collaboratively with the multidisciplinary team to deliver comprehensive assessment, treatment and discharge planning.

Support admission avoidance and timely discharge using a Home First approach.

Provide clinical expertise and work as part of a multidisciplinary team.

Establish and maintain effective communication with patients, relatives, carers and staff.

Communicate complex, sensitive and sometimes distressing information in a clear, compassionate and professional manner.

Support shared decision-making with patients and families, including discussions around risk, discharge planning and ongoing care needs.

Work collaboratively with medical, nursing, therapy, pharmacy, community, social care and voluntary sector colleagues.

Promote effective communication across acute and community pathways.

Chair or contribute to safety huddles, board rounds, multidisciplinary meetings and service meetings.

Develop positive working relationships characterised by trust, respect and open communication.

Promote teamwork and encourage others to contribute to decision-making.

Represent the service at relevant governance, operational and professional meetings.

Promote comprehensive geriatric assessment and holistic care planning.

Ensure risk assessments are completed and acted upon.

Support staff in managing patients with frailty syndromes, including falls, delirium, reduced mobility, polypharmacy, continence needs, cognitive impairment and complex social circumstances.

Ensure safeguarding processes are followed where concerns are identified.

Promote the Home First approach while ensuring risks are clearly identified, documented and escalated.

Support safe discharge planning, including liaison with community services and partners.ortunity where appropriate.
