The post holder will be responsible for the assessment of the individuals in the community/ home/ nursing homes and hospitals to determine eligibility...

The practitioner will utilise the national assessment tools and adhere to the required timescales, co-ordinate, participate and support the assessment process with the multidisciplinary team. The role will be part of the MDT and determine the individuals eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care. This will encompass explaining the process to patients and families; health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.

Qualifications

Essential

  • RGN/ RMN- First Level Nurse
  • Highly skilled nurse competent to practice within the extended role
  • Evidence of on-going post-basic qualifications in area identified
  • Educated to degree Level /or Equivalent (or working towards this)

Desirable

  • Evidence of commitment to lifelong academic learning

Knowledge

Essential

  • Excellent verbal, written and presentation skills.
  • Computer literate- ability to use software programmes to maximise contribution to the post.
  • Demonstrate knowledge of current legislative changes affecting the NHS and the Local Authority.
  • Application of the NMC Code of Conduct and Scope of Professional Practice.

Desirable

  • Project management skills to produce outputs within set timescales

Experience

Essential

  • Minimum of 2 years' experience at Grade 6 level in related field.
  • Demonstrate comprehensive understanding of the Continuing Care and Free Nursing Care Agenda.
  • Proven ability to work across organisational boundaries.

Desirable

  • Operational management in an Intermediate Care setting.

Essential

  • Able to weekends/Bank holidays as per rota
  • Maintain professional integrity
  • Be accessible and flexible to support the needs of the service
  • Good team working (trust and respect each other).
