To assist the CHC Team Administrator in all aspect of administration relating to NHS Continuing Health Care (CHC), Funded Registered Nursing Care (FNC) discharge from hospital and delayed transfers of care and any schemes associated with CHC/CHC bids.
Act as first point of contact for all referrals/enquiries to the CHC Team.
Take telephone messages in the absence of the CHC Team and if necessary signpost the caller to the correct person who can help with their query.
Monitor and ensure correct panel outcomes, application of information to the Broadcare system, liaise with the finance department regarding any change in funding/accommodation etc (Deceased, New Patients, amendments).
Produce reports for the team and CHC Team Managers when required.
Manage the departmental BCU application mailbox and carry out appropriate action.
To maintain and manage electronic diaries for CHC Operational Manager, Team Manager and Nurse Reviewers.
Ensure that letters relating to ratified, deferred or disputed CHC applications are completed and sent out to relevant parties.

Experience
Essential: - Sufficient experience in a busy NHS office environment
Knowledge
Essential
- Understanding of the discharge planning process and Continuing Healthcare Framework
- In-depth knowledge of Microsoft Access, Excel (including formulae), Word, PowerPoint and Publisher to enable the creation of databases, tables, graphs, presentations
Desirable: - Oracle
