The introduction of Care Co-ordinators within the Wyre Bridge PCN is a significant opportunity. This is a role which will evolve along with new functions but will involve working with our practice team and wider multidisciplinary teams as a pivotal role in ensuring all patients receive the best possible care and services available.
You will be responsible for helping to plan and co-ordinate the care needs of our most complex patients.
This role will support the practice in the coordination and delivery of key PCN DES delivery and supporting digital initiatives and responsibilities for ensuring the patient journey through primary care and care transfer across the health system is seamless.
Main Duties and Responsibilities
Act as the first port of call for patients in relation
To their care who are
- Complex
frail or have specific needs
- Frequent GP
appointment attendees
- Recently
discharged from hospital
- Requiring
signposting
Liaise
with multi-disciplinary agencies and secondary care providers to coordinate
pathways of care for patients.
To utilise the
e-referral system for appointments, bookings and admissions as required.
provide an efficient audio, copy typing and word processing service for GPs and
health professionals as required. This includes typing letters, reports,
patient referrals, minutes, emails etc. in an accurate and quality manner.
provide cover for members of the administration team during periods of sickness
and annual leave.
Implementing new referral pathways and training/updating
other staff members accordingly
Provide extensive assistance in arranging correct
clinical pathways for care.
Monitor patient information relating to (for
example), A&E attendance, hospital admission and medications.
with service providers and clinicians to identify frequent flyers, and new
service users utilising risk stratification tools provided and present this
information to the network MDT meetings.
Offer personalised support to patients
and their carers to manage and co-ordinate their ongoing care. Discuss needs with identified
patients and refer where relevant to the PCN social prescribing link worker
service.
Identify patients to discuss at practice and PCN
level with a view to reducing unplanned admissions and exacerbation of
conditions.
Support Quality and Outcome Frameworks and other
National and local targets as required.
Raise awareness of health promotion and NHS health
checks in practice.
Support immunisation programmes
Great Eccleston health Centre is a rural GP practice with an excellent reputation for patient care and service provision.
The Practice is part of a two Practice Primary Care Network with Over Wyre Medical Centre known as Wyre Bridge PCN.
The Practice serves a community of 7900 patients

Qualifications
Essential: - GCSE Grade A to C in English Language and Maths
Desirable: - Evidence of further qualifications e.g. NVQ Level 2-3
Experience
Essential: - Experience of working within a fast-paced responsive environment. Competent in the use of IT systems
Desirable: - Experience of working in Primary Care; - Experience in a health-care related environment; - Experience of Emis Web Clinical system
