  • Work under Physiotherapist guidance to deliver prehabilitation and rehabilitation for people living with and beyond cancer.
  • Manage a caseload across all stages of the cancer pathway.
  • Contribute to MDT meetings, raising and escalating concerns appropriately.
  • Complete administrative duties, including accurate recording, monitoring, and auditing of assessments and outcomes.
  • Oversee waiting lists and caseloads, ensuring smooth patient pathways.Deliver physical activity programmes (1:1 and group) across multiple sites for patients undergoing oncology treatments.
  • Monitor patient progress and adapt programmes to ensure safe, effective care.
  • Promote healthy living and support behaviour change.Provide personalised care using a "what matters to you" approach.Address health inequalities and promote inclusion in service delivery.
  • Work collaboratively with patients, carers, MDT, and external partners.
  • Communicate complex and sensitive information clearly and empathetically.

Qualifications

Essential

  • Level 3 Diploma in exercise referral (CIMSPA accredited)
  • Level 4 Cancer Rehabilitation or willingness to work towards this.
  • Specialist Exercise Instructor Level 4 qualification in chronic conditions e.g. Diabetes, Obesity.

Desirable

  • Courses with relevance to the job e.g., equipment, dementia

Experience

Essential

  • Recent, relevant experience of working in the leisure/ fitness sector with a range of clients including those on an exercise referral scheme
  • Experience of delivering community-based exercise or rehabilitation in both one to one and group settings.
  • Experience of working with people with complex health needs and the impact this can have on exercise
  • Knowledge and understanding of goal setting principles

Desirable

  • Knowledge of behaviour change methods
  • Experience of working with people with cancer
  • Experience in the delivery of cardiac, pulmonary or cancer rehabilitation
  • Knowledge of health inequalities

Knowledge

Essential

  • Understanding of the role of prehabilitation and rehabilitation within Cancer services.
  • Able to recognise when guidance and support is required and initiate appropriate escalation processes
  • Knowledge of the roles of other professionals and the principles of partnership working
  • Knowledge of the impact illness and disability have on rehabilitation

Desirable

  • Knowledge of relevant clinical assessments and treatments
  • Knowledge of the NHS constitution

Skills

Essential

  • Communication and counselling skills, ability to convey sensitive information in an appropriate manner to patients/clients relative/ carers and the MDT
  • Skills to cope with stressful situations, including emotional or aggressive patients and carers
  • Planning and organisational skills and the ability to prioritise own workload and meet required timescales.

Desirable

  • Motivational interviewing skills
  • Skills in the use of digital technology

Other

Essential

  • Current UK driving license
  • Access to own transport
