Job description
Day Webster are currently recruiting for a Band 6 Inpatient Older Adult Discharge Social Worker within Acute Mental Health Services, ensuring effective patient flow and optimal use of inpatient beds. You will be required to work collaboratively with multidisciplinary teams, community services, primary care, partner agencies, and families to identify and overcome barriers to discharge from admission onwards.
