We are pleased to be advertising for a Band 4 Associate Practitioner to work with us in our Enhanced Recovery and Support at Home Team. You will be wo...

  • Deliver high quality care support and assessments in people's homes, under guidance of a registered therapist
  • Undertake initial assessments of new patients as delegated by registered therapists, including early identification of risks and development of appropriate care plans , while promoting patient independence
  • Provide support to the community therapies teams and community rehab wards as needed
  • Be responsible for own caseload, including assessment, rehab progression and discharge planning as delegated by registered therapist
  • Demonstrate capacity to identify, review and risk assess basic equipment needs and issue them in the community autonomously
  • Be able to travel in between patients addresses in a timely manner and organise visits efficiently
  • To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

Qualifications

Essential

  • Foundation degree, NVQ Level 5 or evidence of working towards one of these qualifications, relevant to this role

Skills

Essential

  • Good communication skills , both verbal and written
  • Able to work with minimal supervision and able to escalate appropriately
  • Previous experience undertaking assessments and rehab
  • Confident computer skills with word and patient record systems.

Desirable

  • Knowledge of Risk assessment
  • Competency in performing vital observations

Experience

Essential

  • Previous experience as HCA, Community Support Worker or Therapy Assistant

Desirable

  • Previous community experience
