The Reablement Team assists patients requiring additional

support and rehabilitation on discharge from hospital, and those in the

community who need increased care or therapy input. Interventions and

assessments are holistic, person centred, and focused on maximising

independence. Patients are at the heart of everything we do. This is an

exciting opportunity to work under the guidance of qualified therapists whilst

working closely with other disciplines, gaining knowledge and experience across

a variety of specialities.

As a valued member of the reablement team, you

will work closely with health and social care team members, having the unique

opportunity to experience both sectors. Working under the guidance of qualified

therapists, you will deliver person-centred interventions and support service

users in following tailored reablement programmes. You will manage a caseload

of appropriate reablement cases, regularly reviewing progress, adapting support

plans where appropriate, and maintaining a strong focus on achieving positive

outcomes.

You will be part of a supportive

multidisciplinary team that includes physiotherapists, occupational therapists,

nurses, therapy assistants and social care colleagues. Through collaborative

working, you will help ensure that each individual's needs are met through a

coordinated and holistic approach. Regular clinical supervision and

professional support are provided.

The post

Holder will

Use their initiative and take responsibility for themselves and

the quality of their work and the service they provide to patients.

Act as a role model to colleagues, always seeking to maintain the

highest standards of professionalism.

Hold a small caseload of simple patients under the guidance of Occupational

Therapists and Physiotherapists, co-ordinating the appropriate patient centred

care and reablement plan that promotes independence for the individual.

Be responsible for the timely treatment and regular reviews of patients

who have clear rehabilitation goals under the guidance of qualified therapists.

Work alongside other health and social care colleagues all working

together to provide a patient centred service, aiming to enable service users

to remain safely in their own homes and be as independent as they can be.

Experience

Essential

  • Significant experience working within a relevant clinical service as a senior HCA / HCSW.

Desirable

  • Experience of working at as an Assistant Practitioner or therapy assistant within a relevant clinical service.

Additional Criteria

Essential

  • Ability to travel independently across the county
  • I understand that the Trust will not routinely provide sponsorship to work for this role, and employment on a time limited visa doesn't guarantee sponsorship at point of expiry.

Knowledge

Essential

  • Working knowledge of Assistant Practitioner role within a rehabilitation setting.
  • Understanding of the competency boundaries of an Assistant Practitioner role.

Desirable

  • Has completed Foundation Degree modules within a directly relevant clinical field.

Skills and Abilities

Essential

  • Role modelling good behaviour.
  • Ability to challenge poor behaviour.
  • A positive attitude and able to act on feedback.

Desirable

  • Ability to use IT confidently and competently in daily work.

Qualifications

Essential

  • Foundation Degree in Health and Social Care (or alternative equivalent award e.g. Higher National Diploma (HND) or BTEC (level 4).
  • GCSE Maths and English (Grade A-C) or equivalent.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
