Assertive Outreach Support Worker

• Provide direct practical, physical and psychological support to a small caseload of service users with high levels of complexity.
• Use persistent, flexible and trauma-informed engagement strategies to promote contact with services.
• Supporting with practical skills housing forms/applications appointments.
• Support individuals experiencing disengagement, medication non-adherence, housing instability, substance misuse and increased risk.
• Deliver structured interventions under the supervision of qualified staff, contributing to care planning, risk management and review.
• Work across community settings, including home visits, community-based appointments and remote contact.
• Strengthen partnership working with local hostels as a designated link worker, supporting continuity of care and improving engagement.
• Promote independence, social inclusion and access to community resources.
• Work collaboratively within a supportive MDT, contributing to coordinated care and safe transitions.

• Carry out outreach activities as directed by the Band 6 AO Worker, including home visits, community meetings, and phone/text contact.
• Support the no discharge for DNA approach by using persistent, creative engagement strategies.
• Provide practical support that enables engagement, such as:
• Attending housing appointments
• Supporting with benefits forms
• Helping with ID applications
• Accompanying service users to GP or D&A appointments
• Supporting access to community resources
• Build rapport through empathy, reliability, and traumainformed practice.
• Maintain regular contact attempts in line with the AO model.

• Passionate about working with adults with severe mental illness.
• Skilled in engagement, relationship-building and strengths-based practice.
• Confident working in the community and managing dynamic situations.
• Able to work flexibly, proactively and with persistence.
• Compassionate, reliable and committed to trauma-informed care.
• Experienced in mental health, social care, substance misuse or homelessness pathways (desirable but not essential).
• Eager to learn, develop and contribute to an evolving pathway.

Essential

• NVQ L3 or Foundation degree or equivalent experience.
• GCSE English (A * - C) / National Certificate in Literacy / ESOL Level 2 or equivalent
• Evidence of other healthcare training

Desirable

• Any qualification/diploma related to health/social care/housing support
• Current First Aid Certificate
• Fire Training
• Control & Restraint or Breakaway techniques
• Food Handling / Hygiene Certificate Manual Handling

Essential

• Experience of working as part of a team at Band 3 (or equivalent) with people with acute / severe and enduring mental illness.
• To have had experience in working unsupervised, and in prioritising a workload.

Desirable

• Relevant training in related psychosocial interventions.
