Working with adults who are diagnosed with mental health conditions in secondary mental health care, including: dementia, psychosis, anxiety and depression.Work as part of a multi-disciplinary Community Mental Health Team alongside health and AHP colleagues.Knowledge and experience of implementing the Care Act and Mental Capacity Act is essential.Work directly with service users and their carers using a range of knowledge, skills and interventions as required, developing an individualised approach to the practical and functional needs of the service users.Recognise various mental health problems and undertake or participate in holistic needs based assessments, produce a care plan, implement and review the plan, including the management of risk.Work directly with service users and their carers using a range of knowledge, skills and interventions as required, developing an individualised approach to the practical and functional needs of the service user.Recognise various mental health problems and undertake or participate in holistic needs based assessments, produce a care plan, implement and review the plan, including the management of risk.Work at the service works within the Care Program Approach and sound risk management skills are necessary. Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England.Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)Must have minimum of 3 years post qualifying experience working as a Qualified social worker Benefits of working through Reed, include:
