Unfortunately NPC does not hold a license for Visa Sponsorship and cannot progress any applications for candidates without the right to work in the UK...
Employed by NPC and working within the Norfolk WorkWell service, lead
by Future Projects the Advanced Nurse Practitioner (ANP) provides the senior
clinical oversight of the WorkWell Single Point of Access (SPOA) and chairs the
WorkWell Multidisciplinary Team (MDT). The postholder leads clinical triage of
all referrals, making sure every participant is safely and appropriately
triaged within one working day, that clinical risk (including risk of suicide
or self harm) is robustly considered, and that anyone for whom WorkWell is not
the right service receives a handover under a no wrong door approach.
case discussion, co-ordination and decision making across clinical and
non-clinical colleagues, agreeing clinical input, onward referral, escalation
and discharge so that each participant's WorkWell plan is safe, holistic and
joined up with their GP and wider care. The postholder also practises
autonomously as an advanced clinician: undertaking joint assessments with Work
and Health Coaches, certifying fit notes (Med3) on behalf of primary care, and
providing clinical supervision and education to the team, employers and system
partners. The role requires sound clinical judgement, confident chairing and
facilitation, and a flexible, positive approach to mobilising a new service
across a multi-organisational environment.
1.1 Clinical Triage and Single Point of Access
- Lead clinical triage for the
Norfolk WorkWell SPOA, ensuring all referrals (self-referral, GP and primary
care, employers, VCFSE, Local Authority and DWP partners, education and
Population Health Management case finding) are triaged within one working day
of receipt, against the contractual threshold of 85%.
- Provide clinical oversight and
monitoring of triage completed by non-clinical SPOA colleagues, reviewing
decisions so that clinical risk is robustly considered, and personally triaging
referrals that are clinically complex or flagged as higher risk.
- Determine whether WorkWell is the
most appropriate service against the acceptance criteria in line with the NHS
Constitution.
- Individually assess safety risk
where participants express thoughts of suicide or self-harm (which is not an
exclusion criterion), deciding whether WorkWell is appropriate at that time;
where accepted, ensure care is delivered in partnership with the participant's
GP and/or mental health worker, and where not, facilitate timely transfer to
the right service.
- Ensure referrals that are not
appropriate for WorkWell receive a warm handover to the most appropriate
service, and escalate gaps in provision to the ICB to inform future
commissioning.
- Manage cross-border referrals
with the Suffolk WorkWell SPOA, offering participants choice of where they are
seen and completing warm handovers where appropriate.
- Use access to the participant's
GP clinical record (SystmOne or EMIS) to inform triage decisions and manage
clinical risk, recording triage outcomes accurately with appropriate SNOMED
coding.
- Develop, maintain and review the
WorkWell clinical triage protocol, risk stratification tool and escalation
pathways, aligned to the service's clinical governance framework.
- Lead the quarterly Clinical
Triage Audit required under the contract's local quality requirements,
reporting findings, themes and improvement actions.
- Chair the WorkWell MDT on a
regular scheduled basis (frequency TBC, e.g. weekly) setting the agenda and
ensuring appropriate clinical and non-clinical representation (for example
Occupational Therapists, Mental Health practitioners, Physiotherapists,
Occupational Health professionals, GP input and Work and Health Coaches).
- Facilitate structured case
presentation and discussion by Work and Health Coaches, ensuring each
participant's physical, psychological and social needs are considered through a
biopsychosocial, personalised care approach.
- Lead MDT decision making on
clinical input, joint assessment, onward referral and signposting, escalation
and discharge, ensuring decisions, rationale and actions are recorded
accurately in the participant's record and communicated to those responsible.
- Agree next steps for referrals
returned to the SPOA where contact has not been achieved within five working
days, including contacting the person and the referrer.
- Ensure onward referrals to
delivery partners are supported by due diligence that the partner can meet the
participant's needs, and that support received elsewhere is integrated into
their return-to-work or thrive-in-work plan.
- Review complex, higher-risk or
stalled cases and plans extending beyond the expected intervention period,
ensuring discharge decisions are clinically safe and that the participant's GP
is informed of discharge and outcomes.
- Foster an inclusive
psychologically safe MDT culture in which non-clinical colleagues'
contributions are valued, and use the MDT as a forum for reflective practice
and shared learning.
- Identify themes from MDT
discussion (unmet need, gaps in local provision, skill mix pressures) and feed
these into service improvement, workforce planning and reporting.
- Practise autonomously across the
four pillars of advanced practice (clinical practice, leadership and
management, education and research) in line with the Multi-professional
Framework for Advanced Clinical Practice in England.
- Undertake joint assessments with
the participant's Work and Health Coach where clinical assessment or
intervention is required, so the participant only needs to tell their story
once.
- Deliver evidence-based
low-intensity clinical interventions and self-management support (for example
conservative pain management and stress management advice), embedding Making
Every Contact Count (MECC).
- Certify Med3 fit notes for
participants under WorkWell care in line with DHSC and DWP guidance, updating
the participant's clinical record and communicating with their GP, and
maintaining the required competence (reviewed at least annually).
- Apply mental health competencies
in assessment and care planning, recognising that depression and anxiety are
the most common conditions among the WorkWell cohort.
- Advise participants, Work and
Health Coaches and (with consent) employers on the clinical aspects of
reasonable adjustments, phased returns and changes of duties.
- Refer participants to their GP or
other health professionals where needs go beyond what the core WorkWell MDT can
provide.
- Provide clinical supervision to
Work and Health Coaches and other non-clinical MDT members who deliver
low-level health interventions, ensuring they practise within their competence.
- Contribute to the WorkWell
clinical governance framework so that triage, assessment, intervention and
discharge are clinically safe, evidence based and delivered in line with
professional standards.
- Report and support the review of
patient safety incidents in line with the Patient Safety Incident Response
Framework (PSIRF), sharing themes and learning across the team.
- Act as the clinical point of
escalation for safeguarding concerns arising through the SPOA and MDT, applying
adult and children safeguarding procedures and the Mental Capacity Act in line
with ICB and NPC policies.
- Ensure clinical records are
contemporaneous, accurate and appropriately coded (accepted participants
allocated a SNOMED code), and handled in line with UK GDPR and NPC information
governance policies.
- Provide clinical evidence for the
contract's quality schedule, including triage performance, audit results,
incident themes and participant feedback.
- Build trusted relationships with
GP practices and PCNs, as the main source of referrals, so that referral,
record sharing and fit note arrangements work smoothly.
- Support service evaluation
including baseline and discharge EQ-5D and Attitudes to Work and Health
questionnaires, the 12-week post-discharge follow-up and participant feedback.
- Contribute to the design of
pathways for cohorts at greater risk of health-related economic inactivity,
drawing on population health data and learning from the national WorkWell
pilots.
- Build and maintain effective
relationships with stakeholders across the WorkWell pathway, including (but not
limited to) GPs and practice staff, Primary Care Networks, mental health and
physical health services, VCFSE partners, Local Authority and DWP colleagues,
and employers.
- Handle complex and sensitive
clinical conversations with participants, carers and professionals, including
discussions about risk, capacity to work and onward referral.
- Communicate MDT and triage
decisions clearly and promptly to participants, referrers and GPs, ensuring the
GP is kept up to date on the participant's WorkWell care and outcomes.
- Prepare and present regular
reports on triage performance, MDT activity, clinical risk and audit findings
to the WorkWell Service Manager and relevant governance forums.
2.1 Other Responsibilities
- Working with the WorkWell Service
- Represent the WorkWell Service
Lead at meetings relating to triage, the MDT and clinical safety as required.
- Use service data and clinical
audit to identify variation, risk and opportunities to improve participant
outcomes and experience.
- Facilitate conversations with
partners to bring about changes to pathways based on good practice, participant
experience or variation in performance, in a supportive and non-judgemental
way.
- Use knowledge of local and
national work and health strategies, including the ICB Work and Health Strategy
and the Get Norfolk plan, to develop the service.
- Provide senior clinical cover and
advice to the SPOA during periods of high demand or absence to maintain the one
working day triage standard.

Manager.
