You will assess, diagnose and

treat patients presenting with minor illnesses, providing advice and ongoing

support for their continuing healthcare needs through both face-to-face and

telephone consultations. Home visits are an essential part of this role.

You will deliver a high standard

of patient care as part of a multidisciplinary team of clinicians. This role requires a comprehensive clinical knowledge base, with

the ability to demonstrate safe clinical decision-making and provide expert,

patient-centred care.

You must be able to work both

autonomously and collaboratively as part of the wider practice team.

Clinical Practice

  • Deliver high-quality, evidence-based, patient-centred care to patients registered with the practice.
  • Undertake comprehensive clinical assessment, diagnosis, treatment, management and evaluation of patients presenting with undifferentiated and undiagnosed conditions.
  • Independently manage a wide range of acute, chronic and urgent health conditions within scope of practice.
  • Conduct face-to-face, telephone and home visit consultations as required.
  • Develop and implement appropriate management plans, including medication reviews and referrals to other services where necessary.
  • Request, interpret and act upon diagnostic investigations, including blood tests, ECGs, imaging and other relevant investigations.
  • Recognise serious illness and clinical deterioration, ensuring appropriate escalation and referral to secondary care or emergency services.
  • Provide advice on health promotion, prevention, self-management and lifestyle modification.
  • Support the proactive management of long-term conditions in accordance with local and national guidance.
  • Contribute to reducing health inequalities and improving outcomes for vulnerable patient groups.
  • Maintain accurate, contemporaneous and legally compliant clinical records using the practice clinical system.

Prescribing and Medicines Management- Prescribe safely, effectively and cost-effectively within professional competence and prescribing qualification.

  • Undertake medication reviews and ensure prescribing aligns with local and national guidelines.
  • Monitor patients receiving long-term medications and adjust treatment plans where appropriate.
  • Identify and report medication-related incidents and support safe prescribing initiatives.

Urgent and Same-Day Care- Assess and manage patients presenting with urgent, same-day medical needs.

  • Prioritise clinical risk and ensure timely intervention.
  • Support the management of acute presentations within primary care to reduce avoidable hospital admissions.
  • Liaise effectively with urgent care services, community teams and secondary care providers.

Long-Term Condition Management- Support the identification, monitoring and management of patients with chronic diseases.

  • Contribute to achieving Quality and Outcomes Framework (QOF) and enhanced service targets.
  • Promote preventative healthcare and early intervention strategies.
  • Ensure care planning is patient-centred and supports shared decision-making.

Safeguarding- Recognise and respond appropriately to safeguarding concerns involving children and adults.

  • Maintain awareness of safeguarding legislation, guidance and local procedures.
  • Work closely with safeguarding leads and external agencies where required.
  • Ensure accurate documentation and timely escalation of concerns.

Home Visits and Community Care- Undertake home visits for housebound and vulnerable patients where clinically appropriate.

  • Liaise with community nursing teams, social care providers and allied healthcare professionals to coordinate care.

Multidisciplinary Working- Work collaboratively with GPs, nurses, pharmacists, healthcare assistants, mental health practitioners, social prescribers and other members of the multidisciplinary team.

  • Participate in multidisciplinary team meetings and case discussions.
  • Contribute to integrated care initiatives across the Primary Care Network (PCN).
  • Support seamless transitions between primary, community and secondary care.

Clinical Governance and Quality Improvement- Maintain professional registration and practise in accordance with professional codes of conduct.

  • Participate in clinical audit, quality improvement projects and service evaluations.
  • Contribute to incident reporting, significant event reviews and learning processes.
  • Support implementation of evidence-based practice and national guidance.
  • Assist in developing and reviewing clinical protocols and standard operating procedures.

Digital and Administrative Responsibilities- Utilise clinical systems effectively to support patient care and service delivery.

  • Complete clinical documentation, referrals, correspondence and administrative tasks in a timely manner.
  • Use digital tools and technology to enhance patient access and clinical outcomes.
  • Ensure compliance with information governance, data protection and confidentiality requirements.

Education and Professional Development- Maintain professional competence through continuing professional development (CPD).

  • Participate in mandatory training and appraisal processes.
  • Keep up to date with current clinical guidelines, research and best practice.
  • Support the learning and development of colleagues, students and trainees where appropriate.
  • Contribute to teaching sessions and sharing clinical knowledge within the practice team.

Communication- Communicate effectively with patients, relatives, carers and healthcare professionals.

  • Demonstrate excellent consultation and interpersonal skills.
  • Support patients to make informed decisions regarding their care.
  • Manage challenging conversations sensitively and professionally.

Health and Safety- Comply with practice policies relating to health and safety, infection prevention and control, risk management and emergency preparedness.

  • Promote a safe environment for patients, visitors and staff.
  • Report incidents, hazards and concerns promptly in accordance with practice procedures.

General Responsibilities- Maintain patient confidentiality at all times.

  • Act as a professional role model and ambassador for the practice.
  • Support the practice's commitment to equality, diversity and inclusion.
  • Undertake any other duties commensurate with the grade and scope of the role as reasonably requested by the Partners or Practice Management Team.

Qualifications

Essential

  • Registered ANP (NMC) or Registered Paramedic (HCPC).
  • Primary Care experience.
  • Advanced clinical assessment and diagnostic skills.
  • Experience managing acute and urgent presentations autonomously.
  • Excellent communication and teamworking skills.
  • Commitment to delivering high-quality patient-centred care.

Desirable

  • Independent Prescriber qualification.
  • Minor illness and minor injury experience.
  • Experience using EMIS Web.
  • Experience contributing to QOF, enhanced services and quality improvement projects.
