Advanced Nurse Practitioner

You will provide advanced assessment, diagnosis, and management of patients, working autonomously with GP support. Applicants must be a registered healthcare professional (nurse and paramedic considered) with postgraduate-level training in advanced practice and ideally experience in a general practice setting. The role involves working across the four pillars of advanced practice clinical practice, leadership, education, and research contributing to patient care, service development, and professional growth.

Millwood Surgery is a well-established GP practice serving

approximately 19,800 registered patients. Our multidisciplinary team includes 8

salaried GPs, Advanced Nurse Practitioners, a dedicated pharmacy team, and an

in-house home visiting team, supported by experienced nurses, healthcare

assistants, and administrative staff. We also work closely with colleagues

within our Primary Care Network (PCN), providing additional support from a

wider multidisciplinary team to deliver high-quality, patient-centred care.

Appointments are typically 15 minutes for ACP team, with protected

administration time built into the schedule. Double appointments are available

for patients presenting with more than one condition.

Advanced Nurse Practitioner (ANP)/Advanced Clinical Practitioner (ACP) has been assessed as competent in practice

using their advanced clinical knowledge and skills. They work autonomously and

part of a team to provide decisions in the assessment, diagnosis and treatment

of patients.

ANPs/ACPs in

primary care can develop close, long-term relationships with their patients and

work in partnership with them to achieve optimum health. They are autonomous in

making decisions based on assessment, diagnosis and interpretation of test

results. They can independently prescribe appropriate medication, evaluate or

refer to other specialists if necessary.

ANPs/ACPs are registered nurses with advanced education and clinical training

including non- medical prescribing. They must meet the NMC requirements for

advanced practice, which includes the 4 pillars clinical practice, leadership,

education and research.

The following are the core responsibilities of the Advanced

Nurse Practitioner/Advanced Clinical Practitioner. There may be, on occasion, a requirement to carry out other

tasks; this will be dependent upon factors such as workload and staffing

Develop, implement and embed health promotion

and wellbeing programmes

Provide high-quality and advanced care for

patients of all ages with a range of conditions, including acute and chronic

illnesses. They perform physical exams, take medical histories, and order

diagnostic tests. They also develop and implement care plans, prescribe

medications, and manage ongoing care via face to face, video or telephone

consultations.

Make clinical decisions based on diagnostic

results and adjust treatment plans as needed.

They must demonstrate safe clinical decision-making and expert care. And

work within own scope of practice.

Work with physicians, specialists, and other

healthcare professionals to provide patient-centered care. Provide clinical

triage of patients to appropriate services and healthcare professionals.

Educate patients and their families about

healthy lifestyle choices, disease prevention and self-management techniques.

Stay up to date with the latest evidence and

standards of care through ongoing education and research.

Mentor less experienced nurses and staff.

Support the Practices future development and

share the Practices values.

Manage patients presenting with a range of acute

and chronic medical conditions, providing subject matter expert advice

Identify, manage and support patients at risk of

developing long-term conditions, preventing adverse effects to the patients

health

Provide wound care where appropriate

Provide specialist clinics such as contraception

where appropriate

Undertake the collection of pathological

specimens

Request pathology services as necessary

Process and interpret pathology and other test

results as required

Maintain accurate clinical records in

conjunction with extant legislation

Ensure SNOMED CT codes are used

effectively

Be responsible for the implementation of an

effective immunisation programme

Chaperone patients where necessary

Prioritise health issues and intervene

appropriately

Support the team in dealing with clinical

emergencies

Recognise, assess and refer patients presenting

with mental health needs

Be an extended and supplementary prescriber,

adhering to extant guidance

Support patients in the use of their prescribed

medicines or over the counter medicines (within own scope of practice),

reviewing annually as required

Contribute to practice targets (QOF, etc.),

complying with local and regional guidance

Liaise with external services/agencies to ensure

the patient is supported appropriately (vulnerable patients, etc.)

Delegate clinical responsibilities appropriately

(ensuring safe practice and that the task is within the scope of practice of

the individual)

Support the clinical team with all safeguarding

matters in accordance with local and national policies

Understand practice and local policies for

substance abuse and addictive behaviour, referring patients

appropriatelyProvide chronic disease clinics, delivering patient care as

necessary, referring patients to secondary/specialist care as required

Deliver opportunistic health promotion where

appropriate

In addition to the primary responsibilities, the Advanced

Nurse Practitioner/Advanced Clinical Practitioner may be requested to:

Act as the audit lead, effectively utilising the

audit cycle

Act as the IPC lead for the practice, ensuring

compliance, undertaking audit as necessary

Monitor and ensure the safe storage, rotation

and disposal of medicaments

Participate in local initiatives to enhance

service delivery and patient care

Support and participate in shared learning

within the organisation

Continually review clinical practices,

responding to national policies and initiatives where appropriate

Participate in the review of significant and

near-miss events applying a structured approach i.e., root cause analysis

(RCA)

Develop practice administrative and clinical

protocols in line with the needs of the patient and current legislation
