We are recruiting ACPs on a fixed term contract until 31st August 2027 to join our established and developing Primary Care service within the Rotherha...
This role involves providing high-quality clinical assessment, intervention and treatment
planning for undifferentiated patients, including children presenting to the primary care
stream of the UECC and/or the GP Out of Hours service. You will work autonomously,
making appropriate referrals or discharging patients safely while using advanced
clinical judgement and an evidence-based approach.
You will be expected to work across services, delivering a wide range of consultation
Front-door streaming.
Face-to-face consultations.
Telephone triage and consultations, including patients referred via NHS 111.
Booked clinic appointments.
Home visiting within the local area.
You will also be expected to support the development of trainee ACPs and junior
colleagues, and to contribute to safe, effective service delivery across the wider team.

Experience
Essential
- Experience of working with acutely ill patients
- Experience of autonomous clinical practice
- Experience of multidisciplinary team working
- Experience within primary care, urgent care or emergency care settings
Desirable
- Experience in clinical coordination (e.g. patient flow, prioritisation)
- Experience supporting the education and development of staff
- Experience using clinical systems (e.g. SystmOne,ED systems)
Knowledge
Essential
- Ability to undertake advanced clinical assessment and formulate management plans for undifferentiated presentations
- Understanding of the role and responsibilities of an autonomous practitioner
- Excellent communication skills, including the ability to convey complex or sensitive information effectively
- Strong organisational and prioritisation skills, including the ability to manage competing demands in a busy clinical environment
- Ability to work effectively within and contribute to a multidisciplinary team
- Awareness of current NHS priorities and the urgent and emergency care agenda
- Ability to analyse clinical information and make sound clinical decisions
- Ability to recognise and manage clinical risk and work within own scope of practice
Desirable
- Experience of leadership, supervision or service development
- Experience contributing to quality improvement, audit or change initiatives
- Understanding of local care pathways and community services

Essential: - Current professional registration with NMC or HCPC
Qualifications
Essential
- Degree-level qualification
- MSc in Advanced Clinical Practice (ACP) or equivalent (or working towards for trainee ACPs)
- Non-Medical Prescribing qualification (V300) (or willingness to work towards if trainee)
- Basic Life Support (BLS) certification
- Evidence of ongoing professional development
- Portfolio demonstrating competency as an Advanced Clinical Practitioner
- Ability to participate in an extended-hours rota including evenings and weekends
Desirable: - Teaching or mentorship qualification
