Adult’s Social Worker, Hospital Team, Shropshire Pay rate - £31.97 per hour Complete Care Act Assessments. Undertake Mental Capacity Assessments and Best Interests decisions. Arrange Pathway 3 placements where long-term care is required. Work with individuals, families, and partner agencies to ensure appropriate care and support is in place.
