As an Adult Social Prescriber, you will
- Empower patients to take greater control of their health and wellbeing.
- Connect people with local groups, activities and support services that meet their individual needs.
- Build strong relationships across the voluntary, community and social enterprise sector.
- Develop and deliver wellbeing initiatives, social groups and community-based projects.
- Work as part of a supportive multidisciplinary team to ensure patients receive the right support at the right time.
- Help reduce health inequalities and improve outcomes through a personalised approach to care.
We're looking for someone who
- Has experience supporting people with mental health needs and complex social challenges.
- Is passionate about community-based care and helping people improve their quality of life.
- Has excellent communication, engagement and relationship-building skills.
- Is compassionate, motivated and able to think creatively when supporting patients to overcome barriers.
- Enjoys working collaboratively within a multidisciplinary team while also using their own initiative.
- Is committed to reducing health inequalities and promoting inclusion within the community.
Foundry Healthcare is a large, successful Primary Care Network serving patients across multiple sites in Lewes and the surrounding villages. We are a training practice supported by a strong Partnership and Management Team, alongside a skilled multidisciplinary workforce including Salaried GPs, Advanced Clinical Practitioners, Paramedics, Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Mental Health Practitioners, Nurses and Healthcare Assistants.

Experience
Essential
- Experience in this role
- Experience specifically in a social prescribing, link worker, care navigation or community connector role.
- Knowledge of the local health, social care and VCSE landscape.
- Experience facilitating groups or community activities.
- Respectful of confidentiality and professional boundaries.
Desirable
- Experience using SystmOne
- Experience working in primary care
- Enthusiastic about connecting people with their communities and improving wellbeing.
