Whether you're helping someone remain independent at home, supporting them through a difficult period, or connecting them with services in their community, you'll play an important role in improving lives.You'll join a supportive team within Adult Services, working across a range of settings including community-based services, learning disabilities, hospital discharge and transition services. Every day brings something different, but your focus will always be on helping people achieve the best possible outcomes.This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate. It is also a regulated activity and will be subject to a Children and Adults Barred List check.
