Acute Care At Home Team Lead

Keyelementsoftheroleare: daytodaycoordinationand supervisionoftheteam, providing complex casemanagementandsupport for the team. To be an autonomous practitioner responsible for the assessment and provision of treatments for patients on the caseload. To manage the performance, and clinical effectiveness of the team ensuring the delivery of evidence based care. To be able to provide assurance that performance standards are being met whilst meeting service targets. To identify new opportunities for service development.

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.
