The Senior Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to patients, carers and health and social careprofessionals accessing the Community Nursing Service.
Senior staff nurses will have a minimum of 12-months' community nursingexperience.
The senior staff nurse role supports the Triage Hub team leader with the dailyservice coordination and delivery; and is pivotal in the communication within theteam. The senior Triage nurse is also responsible for providing expert clinicalsupport and guidance to other members of the Triage Team, and those accessingthe service. You will be expected to deputise in the absence of the team leader.
The post holder will be responsible for the assessment of health needs, prevention of hospital admissions and facilitate early discharges from hospital. The current hours of work are Monday to Friday 8am to 4pm, however these will alter to a 7 day per week service, 08:00 - 20:00 as the service expands.

Qualifications
Essential
- RGN
- Current NMC registration
- Community Nursing experience, minimum 12 months
Desirable
- V100/V300 prescribing for nursing or equivalent.
Experience
Essential
- Ability to work collaboratively with ither services and make detailed onward referrals.
- Demonstrate a range of clinical skills
Desirable
- Understanding and /or experience of managing patients with long-term conditions
Knowledge and Skills
Essential
- Knowledge of the of the following areas - Leg-ulcer management, end-of-life care, diabetes, compression-bandaging and wound-management, referrals to other agencies.
- An awareness of current Health and Social Care Policies.
- Demonstrate an understanding of the role of the District Nursing service
- Able to communicate effectively in challenging day-to-day situations.
