To triage incoming referrals assessing the patient's needs, determining the priority of care and processing the referrals accordingly. To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care. The position will require visiting patients in their own environment to provide unplanned nursing care.
We are a small team made up of Triage Nurses, allocators and led by a Clinical Operations Manager and Clinical Lead. We endeavour to provide excellent triaging to ensure that safe, relevant and timely care is provided by the community nursing teams to the local housebound population of West Norfolk. We work closely with our GP and Social Services colleagues and visit patients in their own homes, care homes and clinics as appropriate.
