Transplant Specialist Nurse

Establish nurse led clinics, providing expertise to patient, staff and carers in relation to Transplantation.

Take responsibility for the co-ordination and management of a patient caseload linked to a nurse led clinic, demonstrating high levels of planning and organisational skills. Work as an expert practitioner demonstrating advanced skills in chronic illness management within the speciality of outpatient transplant care and the co-ordination of patient care.

Devise and implement care pathways and protocols following unit guidelines to enhance patient care. In conjunction with the transplant team develop and implement a transplant strategy with the aim of improving patient care and promoting the further development of nursing within the speciality taking into account audit, patient and staff feedback and Trust policies, and ensure appropriate dissemination.

Be responsible for a caseload of patients identified for nurse led follow up clinics by consultant teams, including physical clinical assessments where appropriate. Select and order laboratory/screening tests for patients within unit protocols according to best practice and to individual need.

Review these and other medical test results interpreting results and deciding upon appropriate nurse led action with referral to medical team if required. Be able to respond knowledgeably and appropriately to normal and abnormal biomedical results, and communicate with medical/surgical staff, and keep patients informed of changes, ensuring the renal database is updated Monitor blood results and medication doses for identified patients within designated protocols.

Daily review of blood results/investigations of transplant patients attending outpatient clinics. Liaise with medical staff and GPs regarding management according to unit protocols. Review and manage patient care pathways according to unit protocols. Develop and utilise a thorough understanding of relevant unit policies, procedures and protocols in order to both review and manage care pathways and advise colleagues
