This is a patient facing role and as a Transitional Worker your responsibilities will vary. You will:

  • work collaboratively with the patient and other agencies to support a safe transition of care from custody to community
  • manage the assessment of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment
  • manage key risks associated with resettlement in the prison population
  • provide support in the community, helping support their needs for hospital/ GP registrations/ appointments
  • attend the complex case release meeting which is a multiple disciplinary meeting that includes representatives from safer custody, healthcare and external services

We have an exciting opportunity for a Transitional Worker Administrator to join team at HMP Brixton a very busy prison in London.

Hours

This is a full time role working Monday to Friday, 8.30am to 4.30pm.

Salary

Youll receive an annual salary from £27,000 to £33,000 per annum - depending on experience.

Qualifications, skills and experience required

  • Experience of working in a prison, Mental Health or homelessness environment would be beneficial
  • Administration experience
  • Experience of working in multi-agency teams
  • Experience of working with charities and similar organisations
  • Knowledge of criminal justice systems in England
  • Excellent communication skills and ability to communicate openly, consistently and clearly
  • Proactive problem solving skills
