Support Worker

Carry out comprehensive support assessments, risk assessments, and develop person-centred support plans.Provide ongoing one-to-one support to service users, regularly reviewing support plans and outcomes.Assist residents to access health, education, welfare, and community services, including attending appointments where required.Support individuals to maintain their tenancy and develop independent living skills.Identify safeguarding concerns and ensure appropriate action is taken in line with policies and procedures.Maintain accurate case notes, support records, and monitoring information.Build positive relationships with external agencies and partner organisations to ensure coordinated support provision.Encourage engagement in meaningful activities that promote wellbeing, confidence, and independence.

Previous experience supporting adults within a housing, supported living, homelessness, mental health, or similar support setting.Experience completing risk assessments and support plans.Strong understanding of safeguarding and supporting vulnerable adults.Excellent communication and interpersonal skills.Full UK driving licence and access to a vehicle.Enhanced Child and Adult DBS check (or willingness to obtain).
