Provide practical and emotional support tailored to individual needs.Assist with personal care and daily living activities where required.Support individuals to access appointments, social activities, and community services.Promote independence, choice, dignity, and inclusion.Maintain accurate records and follow support plans.Work collaboratively with colleagues, families, and healthcare professionals.Adhere to safeguarding, health and safety, and company policies at all times. Essential Requirements

Previous experience as a Support Worker, Care Assistant, or Healthcare Assistant.Up-to-date mandatory training certificates.Practical Moving and Handling training certificate (essential).Strong communication and interpersonal skills.A caring, patient, and professional approach.Enhanced DBS registered on the Update Service (essential).Right to work in the UK.Flexibility to work shifts, including evenings and weekends where required.
