Providing practical and emotional support to individuals in line with their personal care plansAssisting with daily living activities including personal care, meal preparation, and medication supportSupporting individuals to maintain their independence, dignity, and quality of lifeEncouraging participation in social, recreational, and community activitiesBuilding positive, trusting relationships with the people you support and their familiesMaintaining accurate and up-to-date records and documentationWorking collaboratively with colleagues, healthcare professionals, and other agencies to deliver the best possible outcomes What We Are Looking For
