- Work closely as part of the Multi Disciplinary Team for the care of patients requiring medical and rehabilitation support, predominantly post and acute hospital stay.
- Responsible for the assessment of care needs, development and implementation of patient centred care. Promoting independence and safe discharge home.
- Working with junior members of staff in all aspects of patient care, participating in training and support.
