  • To deliver direct and indirect patient care, focussing on local needs and priorities addressing the prevention agenda, care centred on what matters to the person, via personalised care and support planning"
  • To participate in the assessment, development, implementation and evaluation of programmes of care. Elderly Virtual ward is a 24 hour service and, as such, an element of flexible working is expected to maintain safe, effective, responsive services.

You are also required to have a driving licence and your own vehicle, which needs to be available for work

Elderly Virtual Ward is an exciting and rewarding challenge where you will gain experience in a vast range of clinical skills to help you along your journey as a nurse. You will be supported by an expert team where training and development opportunities are actively encouraged in order to meet the clinical needs of the role. Support and development will be individualised depending on your current skills and experience.

Experience

Essential

  • Work as a Band 5 nurse in the community setting
  • Clinical experience in the speciality of the post (frailty)
  • Experience of completing Comprehensive Geriatric Assessment, IVs, catheter, wound care, rehabilitation, enablement
  • Experience of working as part of a team with the ability to prioritise own workload
  • Experience of working with patients, and their network from a range of diverse backgrounds and cultures
  • IT literacy and ability to use NHS systems including SystmOne and EPR

Skills

Essential

  • Excellent interpersonal and communication skills (face to face and written) with the ability to adapt to meet the requirements of patients, their support network and professional colleagues.

Desirable

  • Skills in research and audit methodology and application of findings to practice

Knowledge

Essential

  • Understanding of Information Governance and Confidentiality

Qualifications

Essential

  • Full UK Driving License
  • Practise Assessor/Supervisor qualification or willingness to complete

Desirable

  • Evidence of CPD
  • Additional post-registration qualification relevant to field of practice
